Gas Station Testing Forms
Here you will find information about:
The following forms can be used in the state of Washington or
Oregon for compliance testing of vapor recovery equipment
installed at gasoline dispensing stations.
The forms are provided in PDF format. This means
you will need the Adobe Acrobat Reader in order to print the
forms once you have downloaded them. If you do not have the
Acrobat Reader, please go to the Adobe web site to download this
free software.
Downloading these documents is easy. If you have a good
host connection, it should only take a couple of minutes to
download them.
- Pressure Decay test form is
used to record the CARB system certification test results
from a static performance test (TP-201.3 or Exhibit 3
test procedures) of the tank and related stage 1 and 2
vapor recover equipment.
- Back Pressure test form is
used to record the CARB system certification test
(TP-201.4) results of a dynamic performance test on stage
2 vapor piping,
- Air to Liquid Ratio test form
is used to record the CARB system certification results
(TP-201.5) of the air to liquid dispensing ratio (by
volume) for vacuum assist stage 2 systems.
- Air to Liquid Ratio-Alternate Method
VacuChek form is used when a VacuChek testing
equipment is employed to test the Air to Liquid Ratio of
stage 2 vacuum assist systems.
- Air to Liquid Ratio -Alternative
Method VacuSmart form is used when VacuSmart testing
equipment is employed to determine the Air to Liquid
Ratio of stage 2 Vacuum assist systems.
- Compliance Testing Matrix lists
the California Air Resources Board (CARB) executive
orders and the required tests that should be conducted on
stage 1and 2 systems.
- Testing Contractor Notification
form should be used by the gas station
compliance-testing contractor to give advance
notification to the local air authority of impending
compliance tests.
- Healy Return Line Tightness test
form is used to record CARB system certification
results for the Healy Stage 2 Vapor Return Line Vacuum
Integrity Test.
